There are two kinds of Google Alerts, one is a private company [http://www.googlealert.com/] using Google's commercial api and the other is a free service from Google themselves.
Whenever there is a new search url that shows up in Google's search results for your query, you'll receive an alert... that's pretty much it.

Maybe you are not understanding me Jason, this post you made was emailed to me via google alerts for "Matt Cutts". I have a handful of blogs but never appear in Google Alerts sent via email, you do so I wanted to know how and why, thanks. :)

Because we're syndicated in Google News and those get put into the Alerts database immediately if they match the query string you're looking for.
